Individualized Home Supports


 

What is Individualized Home Supports?

The Individualized Home Supports (IHS) Program was developed to provide quality, one-on-one life skills training in the home and community. IHS provides extra support where needed to help the person served maintain their home, build healthy social relationships, and continue living independently. The program was designed to coach, train, and support individuals served in moving forward in life and reaching their full potential.

Our IHS program focuses on each individual's spiritual, mental, emotional, physical, and social well-being, and our trained staff assists each person in developing a care plan with personal goals (“outcomes”) in these five key areas of growth.

Who do we serve?

The IHS Program is available to adults (ages 18 and older) who have been diagnosed with a physical or neurological disability, brain injury, and/or mental illness. An initial interview and assessment will help determine what supports may be provided by the program.

How do i get started?

Step 1: Review admission criteria to check if the person served qualifies for services

Step 2: Fill out a program application

Step 3: Once the application is received, a “Meet and Greet” will be set up

Step 4: Applicant will be put on the waiting list

Step 5: Once an applicant has reached the top of the waiting list and qualifies for IHS services, an IHS staff member will reach out to set up an intake meeting

 

Admission Criteria

  • Is physically, mentally, and medically safe to participate in programming

  • Does not have a communicable disease

  • Does not cause harm to self or others

  • Has no unmanageable wandering

  • Is able to be redirected

  • Is continent or has manageable incontinence

  • Is mobile with or without a device

  • Can transfer by his/her self

  • Is not actively abusing drugs or alcohol

  • Has set goals and is motivated to achieve those goals

  • Is motivated to participate in all areas of programming
